Required Forms

All required forms must be filled out and returned to the athletic department
BEFORE student-athletes may participate.

1. Health Examination and Consent Form:

(this is required for 6th grade XC, all 7th graders, all 9th graders, all 11th graders OR for those students who haven't completed them). 

2. Interim Health Questionnaire Form:
